A Case of Osteomalacia and Normocalcemic Primary Hyperparathyroidism A 43 year-old female patient had difficulty in walking, low back pain and pain in hips and shoulders. Decreased serum phosphate and 25 hydroxyvitamine-D concentrations and normal level of serum calcium were found. The serum PTH and ALP levels were elevated. Skeletal x-ray examination revealed osteopenia, bone cyst-like lesions of various size and pseudofractures. Her whole body bone scan demonstrated enchanced uptake throughout the skeleton together with multiple focal abnormalities (pseudofractures). A parathyroid adenoma was demonstrated by echography and scan. After adenoma excision, biochemical and radiologic abnormalities were normalized. Clinical picture of the patient improved without any symptoms.
