Quadriceps muscle weakness is common in knee osteoarthritis patients and adversely affects functional capacity. In this study, effects of concentric-concentric (con-con) (quadriceps-hamstring) versus concentric-eccentric (con-ecc) (quadriceps-quadriceps) isokinetic training programs on pain, functional capacity and quadriceps muscle strength were investigated in patients with knee osteoarthritis. Twenty female patients with knee osteoarthritis according to American College of Rheumatology (ACR) diagnostic criteria and with a mean age of 54.2±6.4 years were distributed into con-con and con-ecc training groups each containing 10 patients. Exercise programs were carried out in both groups at the same five angular velocities by using a computerised isokinetic test and exercise system for 10 times and completed in 2 weeks. In the post-treatment evaluation pain scores (VAS) and extensor and flexor peak torque values at 180°/second angular velocity were improved significantly in con-con group whereas both pain and WOMAC scores and extension peak torque values at 90 and 180°/second angular velocities were improved in con-ecc training group (p<0,05). In conclusion we can say that choosing concentric-eccentric training programs in knee osteoarthritis patients should be worth-while because of the rapid improvements achieved in pain, functional capacity and quadriceps strength in changing velocities by this type of exercise programs.
