There are many certain sistemic diseases that may be associated with clubbing of fingers. In primary (idiopathic) hypertrophic osteoarthropathy (HOA) which is a rare condition, there are marked skin signs in addition to clubbing. For this reason, the definition of primary HOA as pachydermoperiostosis is largely accepted. In patients with primary HOA, the hypertrophy of the skin is so marked that, thickening of facial folds and appearance of leonine face may suggest acromegaly. In this article a patient with leonine face due to pachydermoperiostosis is presented and discussed along with literature review.
