Seronegatif Spondiloartropatilerde HLA-B27 ve Klinik Bulgularla İlişkisi

It has long been recognized that HLA-B27 constitutes risk factor for eye and cardiac involvement, and increases susceptibility to seronegative spondyloarthropathies. In this study, we searched the relationship between HLA-B27 and the seronegative spondyloarthropathies in patients admitted to our rheumatology department. Fifty-seven patients were included in the study. There were 43 patients with ankylosing spondylitis, 9 patients with reactive arthritis, 4 patients with juvenile chronic arthritis, and 1 patient with psoriatic arthritis. There were 12 female, and 49 male patients. Age of the patients were ranging between 9 and 72 years (mean±SD: 34.89±10.55 years). A detailed history was taken and a complete physical examination was done for all of the patients and HLA-B27 tissue antigen was studied. The family history, extra-articular, and peripheral joint involvements were evaluated. HLA-B27 was positive in 28 of the 57 cases with spondyloarthropathy (49.12%). It was positive in 25 of the 43 ankylosing spondylitis patients (58.1%). Family history was positive in 12 patients. There was peripheral joint involvement in 19 patients and extra-articular involvement in 10 patients. There was not a statisticaly significant difference between the HLA-B27 positive and negative cases regarding family history and peripheral joint involvement, whereas extra-articular involvement was found to be more frequent in HLA-B27 positive patients (p<0.05). In conclusion; among the seronegative spondyloarthropathy patients admitted to our clinics, the positivity of HLA-B27 was found to be lower than those previously reported, especially in those cases with ankylosing spondylitis. Extra-articular involvement is more frequent in HLA-B27 positive cases and this should be taken into consideration in the follow-up of the patients.
